Do I still use insurance?

Yes. Your existing insurance can still be used for many medical services.

The membership fee covers the physician’s time, access, and preventive care oversight, which are typically not reimbursed through insurance.

However, medically necessary services such as laboratory testing, imaging, or specialist referrals may still be billed to your insurance if they are covered under your plan.

Many members find this structure beneficial because it allows their physician to focus on preventive care and personalized treatment decisions without the restrictions often imposed by insurance reimbursement models.

What happens if I need a specialist?

When specialty care is needed, your physician coordinates the process directly.

This includes helping determine the appropriate specialist, arranging diagnostic testing when necessary, and communicating clinical findings to ensure continuity of care.

Because your physician maintains an ongoing understanding of your health history, risk factors, and diagnostic results, referrals can be more targeted and efficient than traditional referral processes.

How do I schedule an appointment?

Members can schedule appointments directly through our secure member portal or by contacting the office.

Because enrollment is limited, appointments are typically more flexible and responsive than traditional primary care scheduling.

Depending on membership tier and availability, many members are able to schedule same-day or next-day visits when needed.

The program is designed to provide convenient physician access and ongoing communication, allowing medical questions or concerns to be addressed before they become larger issues.

PROFESSIONAL DISCLOSURE

Khan Longevity Care provides preventive and primary medical services. All diagnostic and treatment decisions are individualized and evidence-based. Membership does not replace emergency or specialty care when indicated.

ADDITIONAL TERMS:

Services Fees cover Practice’s annual and follow-up routine diagnostic exam services, supported by year-long expansive communications with your healthcare professional will optimize your achievable healthcare goals. Practice’s reduced patient panel, and detachment from dependence on Plan reimbursement for Plan services that often include restrictions or limits on those Plan services, all enable Practice to provide expedited and unhurried exam and communication services. All services paid for by Services Fees are entirely outside of and beyond any Plan coverage. Services Fees must therefore never be submitted to Medicare or any other Plan for reimbursement.

Practice may separately elect to deliver Plan-covered services when medically indicated if Practice is in-network with the Plan. Plan-covered services will be submitted to the Plan for reimbursement, and Plan-mandated co-payments/deductible charges shall be collected from Patient by Practice. Services Fees do not cover such Plan-covered services or Plan-mandated Patient charges.

Services and Services Fees do not include any Medicare-covered services. However, Medicare-eligible patients may request and receive the Welcome To Medicare Physical, the Annual Wellness Visit, chronic care management/CCM services, remote patient monitoring/RPM services, and other Medicare-covered services. Practice must bill Medicare for these services and must collect Medicare-mandated co-payments, deductibles, and other Patient charges related to Medicare-covered services.
